Capital Nephrology Welcomes- Transient Patient Information
Capital Nephrology Associates, PA offers Visiting Dialysis Patient services to persons who will be visiting to the Raleigh and Surrounding Triangle area. If you would like to schedule treatment with us, you will need to download and print a Transient Patient Form for you and your physician. Once completed by you and your physician, you can fax it to our administrative office, (our fax number and phone number are included).
Appointments
Office hours are Monday through Thursday 8 a.m. to 5 p.m. Office hours on Fridays 8 a.m to 4 p.m.Our office will call 24-48 hours priorto confirm your appointment.
Special scheduling can accommodate unusual circumstances.
No Show & Cancellation
24 hours notice requested for appointment cancellations
A $25 fee will be applied to your account for NO SHOW appointments
Accessible To You 24 Hours a Day
Our physicians are always available for 24 hours a day, 365 days a year. Call our office, speak with our answering service and one they will contact the physicians on call. If you have an emergency, that can not wait for a return call please go directly to the emergency room.
Insurance
We participate in most major HMO and PPO plans. We are a participating provider for Medicare and Medicaid. You are asked to bring your insurance card to EACH appointment. We also ask you to provide us any information regarding changes in your insurance coverage or your address changes.
ALL PATIENTS ARE EXPECTED TO PAY YOUR CO-PAYMENTS AT THE TIME OF SERVICE.
We accept cash, checks, MasterCard and VISA.
If you are a SELF PAY PATIENT we have several payment plans available.
Hospitalization
Hospitalization can cause considerable anxiety to anyone. While hospitalized your primary physician may request a nephrology consultation, in this case, our staff will visit you, as needed, to complete recommendation for further diagnosis and treatment. We will explain the cause and treatment of your condition as well as the risks and benefits of each procedure ordered. If at any time your status or illness is unclear to you, please let us know. We will make every effort to answer your questions.
Medications
Please bring your medicines with you to EACH appointment.
When you are in need of a prescription refill, please contact your pharmacist and have them fax a refill request to the office at 919-231-3912. There is typically a 48 hour turn around time for prescription refills.
We DO NOT call in narcotics. We reserve the right not to treat or diagnosis a patients over the phone without the benefit of physical exam and medical record.
Medical Records & Forms
There is a nominal fee for filling out any type of disability documentation. Your medical records are available upon a request.
